New Assessment - Phase 1A Planning and Preparation

After filling in the initial information, you will be redirected to Phase 1A Planning and Preparation.





Facility/Site Information

Enter the information of the facility/site you are assessing in the Facility/Site Information section.




  1. The 'Facility/Site Name' field has been pre-filled with the facility/site name that you previously filled in. But you can still edit the facility/site name.
          
  1. Input the facility/site business identification number in the 'Business Identification Number' field. This is a mandatory field.
          
  1. The ‘Company ID’ field is currently empty and disabled for editing. Once you save the data or proceed to the next phase, SIRI will generate a company ID. You can then use the company ID as the Parent Reference ID or Company Reference ID in the Pre-assessment form for your next assessment.
          
  1. If the facility/site has a parent business, you can add it by clicking on the 'Add Parent Facility/Site Details' button.
          

          A pop-up window will appear.

          

          Select the organisation type. If it is a parent company, then choose the ‘Parent Company’ option. If it is a subsidiary company choose the ‘Subsidiary Company’ option.

          
          You can select the parent company/subsidiary company from your previous assessment by switching the 'Select from previous assessment' toggle.
         
          
          A list of previous assessments will appear. If this is your first assessment, there will be no previous assessments.  

          
          If this is not your first assessment, you will see a list of your previous assessments. You can select the parent/subsidiary company in the 'Select previous assessment' field. This is a mandatory field.
          

          If the parent/subsidiary company is not in your previous assessment, you can add the company name in the 'Company Name' field. Add the parent/subsidiary business identification number in the 'Business Identification Number' field. Both fields are mandatory.

           

          After that, click on the 'Add Parent' or 'Add Subsidiary' button to add the parent/subsidiary company.

          

          Then the parent/subsidiary company will be shown in the Parent/Subsidiary table.
          
         
      
          You can add both parent and subsidiary company to the facility/site that you are assessing. You can add more than one subsidiary company but only one parent company.
          

          You can also delete the parent/subsidiary company by clicking on the  icon.
          

Address Information

Enter the address information of the facility/site you are assessing in the Address Information section.



  1. Input the address label in the 'Address Label' field. This is a mandatory field.
          
  1. Select the location type in the 'Location Type' field. This is a mandatory field.
          
  1. Input the facility/site address in the 'Address One' field. This is a mandatory field.
          
  1. Input the facility/site's other address in the 'Address Two' field.
          
  1. Input the city where the facility/site is in in the 'City' field. This is a mandatory field.
          
  1. Input the state where the facility is in in the 'State' field. This is a mandatory field.
          
  1. The 'Country' field is disabled for editing and is automatically filled from the pre-assessment form.
          
  1. Input the postal code of the facility/site in the 'Postal Code' field. This is a mandatory field.
          

Industry Classification

Select and input the industry classification of the facility/site you are assessing in the Industry Classification section.



  1. Select the industry of the facility/site by clicking on the 'Select Industry' button.
          

          A pop-up window will appear. You can use the search bar to find industries and make your selection easier.
          

          After that, select the industry then click on the 'Submit' button.
          

          After selecting the industry, the industry cluster, industry sector, and industry subclassification will appear.
          
  1. Select the facility/site ownership type in the 'Facility/Site Ownership Type' field. This is a mandatory field.
          
  1. Input the facility/site age in the 'Facility/Site Age (In Years)' field. This is a mandatory field.
          
  1. Select the facility/site currency in the 'Facility/Site Currency' field. This is a mandatory field.
          
  1. Input the annual manufacturing output in the 'Annual Manf. Output' field. This is a mandatory field.
          
  1. Input the number of employees in the 'Employee Count' field. This is a mandatory field.
          

Subsidies

If the facility/location that you are assessing receives subsidies, toggle the 'Subsidies' switch. If not, you can skip this section.



Click on the 'Add' button to add a subsidy. A new section will appear. You can also add more than one subsidy.



  1. Select the subsidiary information from the 'Subsidiary Information' field.
          
  1. Input the subsidiary name in the 'Subsidiary Name' field.
          
  1. Input the subsidiary amount in the 'Subsidiary Amount' field.
          
  1. You can delete the subsidiary by clicking on the  icon.
          

Executives Involve

The Executive Involve section will be automatically filled with the key contact person information provided in the pre-assessment form pop-up. But you can still edit the executive’s involved information.



  1. Select the title of the executive/contact from the 'Title' field. This is a mandatory field.
          
  1. Input the executive/contact first name in the 'First Name' field. This is a mandatory field.
          
  1. Input the executive/contact's last name in the 'Last Name' field. This is a mandatory field.
          
  1. Input the executive/contact designation in the 'Designation' field. This is a mandatory field.
          
  1. Input the executive/contact email address in the 'Email' field. This is a mandatory field.
          
  1. You can add more than one executive/contact by clicking on the 'Add' button.
          
  1. You can also upload an Excel file to fill out the executive/contact information by clicking on the 'Upload Excel' file.
          
          A pop-up window will appear. You can upload the excel file by clicking on the 'Upload' icon or drag and drop the file to the 'Upload' pop-up window.
          

          Or you can download the excel template that has been provided by clicking on the 'Download Excel Template'. You can edit the template with the executive/contact from the facility/site that you are assessing.
          

          After that, click on the 'Submit' button.
          

          All the executive/contact from the excel file will appear at the Executives Involve section.
          
  1. If there is more than one executive/contact, you can choose the primary contact by clicking the contact box.
          
  1. You can also delete contacts, other than primary contact, by clicking on the  icon.
          

Assessment Scope

Enter the assessment scopes for the facility/site that you are assessing in the Assessment Scope section.




  1. If the facility/site that you are assessing is manufacturing a product, tick on the 'Manufactures the Following Product' option and input the product in the ‘Product’ field.
          
  1. If the facility/site that you are assessing is providing a service, tick on the 'Provide the Following Service' option and input the service in the ‘Service’ field.
          
  1. You can choose both options if the facility/site that you are assessing is manufacturing a product and providing a service.
          
  1. Choose the scope of the assessment. If the assessment is applied to the entire factory/plant, then tick on the ‘Covers the Entire Factory/Plant’ option.
          
  1. If not, then tick on the ‘Covers the Following Sections of the Factory/Plant’. Then input the following section of the factory/plant/ facility that you are assessing in the ‘Mention Here’ field.
          
  1. If you are assessing your own company or any company that is directly or indirectly associated with you, select the 'Assessing own or any company' option. You will not receive a SIRI emblem in your report document when assessing your own company or any company that is directly or indirectly linked to you. If this does not apply, you can leave this field empty.
          

Assessment Dates

Enter the assessment dates for the facility/site that you are assessing in the Assessment Dates section.



  1. Select the assessment date of the onboarding calls in the 'Date of Onboarding Call' field. This is a mandatory field.
          
  1. Select the date of evaluation when the assessment will be conducted in the 'Date of Evaluation' field. This is a mandatory field.
          
  1. Select the assessment date of debrief in the 'Date of Debrief' field. This is a mandatory field.
          

If you want to save facility/site information without filling in all mandatory fields, click on the 'Save Data' button. You can continue filling in facility/site information later without losing your previously saved data.





If you want to continue to the next phase, click on the 'Next Phase' button. Then you will be redirected to Phase 2A – Evaluation (Assessment Matrix).





SIRI will also simultaneously send primary contact a credential email for the Client portal. This will allow the primary contact to monitor the assessment progress. If the primary contact does not get the credential, you can send it again by clicking on the ‘Send Assessment Credential Email’ hyperlink in the Executives Involve section.
