New Assessment - Initial Information
To create a new assessment, under the Active Assessment or Draft Assessment menu click on the 'Create New Assessment' button located at the top right side of the page and select 'New Assessment'.
Then a pre-assessment pop-up form will appear.
- The ‘Country’ field is automatically filled in with the country you choose when you verify your profile details the first time you log in to the SIRI portal. But if the country where the assessment is conducted is different, you can choose the country in the 'Country' field. This is a mandatory field.
- Input parent facility/site company ID in the ‘Parent Company ID’ field. Parent Company ID is a company ID to the parent company which has previously undergone a SIRI Assessment. All assessments with the same Parent Company ID will be consolidated under this identifier. You can get the parent company ID from the previous assessment you conducted. If you don’t have any parent company ID, you can leave this field empty.
To see the parent company ID from the previous assessment, go to your previous assessment and click on the Phase 1A – Planning and Preparation. The parent reference ID will be in the Company ID field under Facility/Site Information section.

- Select the assessment reference in the ‘Assessment Reference’ field. This is a mandatory field.
- If you choose Facility/Site in the ‘Assessment Reference’ field, input the facility or site name where the assessment is conducted in the ‘Facility/Site’ field. This is a mandatory field.
- If you choose an Existing Company in the ‘Assessment Reference’ field, input the existing company ID in the ‘Company ID’ field. This is a mandatory field. Company ID is a unique identifier assigned to the related company which has previously undergone a SIRI assessment. All assessments with the same Company ID will be consolidated under this identifier. You can get the company ID from the previous assessment you conducted.
To see the company ID from the previous assessment, go to your previous assessment and click on the Phase 1A – Planning and Preparation. The company ID will be in the Company ID field under Facility/Site Information section.

icon to learn more about Company ID. - Select the title for the key contact person in the ‘Title’ field in the Key Contact Person section. This is a mandatory field.
- Input key contact person first name in the ‘First Name' field in the Key Contact Person section. This is a mandatory field.
- Input key contact person last name in the ‘Last Name' field in the Key Contact Person section. This is a mandatory field.
- Input key contact person designation in the ‘Designation’ field in the Key Contact Person section. This is a mandatory field.
- Input key contact person email in the ‘Email' field in the Key Contact Person section. This is a mandatory field.
- A warning message will appear to remind you to complete the assessment by the deadline set by the system. This is to ensure that you are able to complete the assessment within the given timeframe.
- If you are assessing a facility or site that is your own company or is linked to you directly or indirectly, tick the 'Assessing own company' checkbox.
- Tick on the 'Declaration form' checkbox to imply your agreement to comply with INCIT declaration form for assessment creation.
When you tick this checkbox, a new pop-up will appear to display the declaration form. Click on the 'Confirm' button to approve the declaration form. This is a mandatory field.
After that, click on the 'Submit' button to start your assessment to the company. Or click on the 'Cancel' button to cancel creating a new assessment.
After you have submitted the initial information:
- If you are an individual assessor, you must make the payment for the assessment first. To learn more about the payment process, please visit this link: Payment for Individual Assessors
- If you are a company assessor or an assessor with a company contract, you can continue your assessment and you will be redirected to Phase 1A - Planning and Preparation.
